Malad High School Athletes

When it is time for sports physicals here is what you need to know.

Q: Who needs a sports physical?
A: Any incoming Freshman (9th), Junior (11th) or if it has been 2 years since your last sports physical or any new athlete.

Q: Where and when will physicals be conducted?
A: Physicals will be conducted at the Oneida County Clinic immediately prior to the start of a new school year with appointment times typically from 8am-5pm. Please call (208) 766-2600 to make an appointment.

Q: What do you need to bring to the physical with you?
A: A completed IHSAA Exam and Consent form and if under 16 years of age you must be accompanied by a parent or guardian.

Q: Where is IHSAA form available?
A: It can be downloaded here
Idaho High School Activities Association Idaho Health Examination and Consent Form and it will also be available at the clinic office and hospital reception desk prior to the physical as they MUST be completed prior to the physical.

Q: Can the physical be billed to insurance?
A: No, the physical will not be billed to insurance and the associated fee will need to be paid at the time of the service.
